Care Management Program
You can refer Health New England members with complex care needs to our Integrated Care Management (ICM) Program. Members will have the opportunity to interact directly with a Registered Nurse who will work with the member to improve specific health related goals. Available programs include but are not limited to: Diabetes, Asthma, Coronary Heart Disease, and High Risk Maternity. The Integrated Care case managers provide members with one direct contact at HNE for medical concerns; this design allows them to build trusting relationships with HNE members and treating providers. Quality Management Program
HNE has a written Quality Management Program Description. This document provides detailed information about the program and contains the annual work plan, or schedule of events.
