Prior Authorization Requirements – August 2021

Effective October 1, 2021, the following services will now require a Prior Authorization. Previously, these services were reviewed post adjudication for medical necessity and coverage under the terms of the member’s plan (i.e., not cosmetic or non-covered treatment).
