Coming soon, Health New England will be working with Healthmap Solutions, Inc. (Healthmap) to provide care coordination services for Medicare Advantage members with chronic kidney disease (CKD) or end-stage renal disease (ESRD).
Healthmap is a growing specialty Population Health Management (PHM) company with an industry-leading Kidney Health Management (KHM) program that will greatly benefit our providers and members.
