New: Mom’s Meals NourishCare for Medicare Advantage Members

Effective 1/1/20, our Medicare Advantage members will have the opportunity to be referred to Mom’s Meals NourishCare® for complete nutritional meals after discharge from an inpatient hospital stay.
Here is how it works: Following a hospital stay, our Care Management team will work with the member to select a meal from available categories such as General Wellness (low sodium, heart friendly, vegetarian), Diabetes-Friendly, Renal-Friendly, Gluten-Free, Pureed and attention to food allergies. Once completed, the form will be submitted to Mom’s Meals and, if approved, two meals a day for 14 days will arrive packed in a cooler each week.
This benefit is as easy as it sounds and allows our member to focus on getting better after returning home from the hospital.
