Medicare Advantage Members Mom’s Meals (Reminder)

In October, we shared a new benefit for 2020. Our Medicare Advantage members have the opportunity to be referred to Mom’s Meals to receive 28 fully-prepared, nutritional meals after discharge from an inpatient hospital stay.
Within 30 days after discharge, our Care Management team will reach out to the member to select meals from available categories such as General Wellness (low sodium, heart friendly, vegetarian), Diabetes-Friendly, Renal-Friendly, Gluten-Free, and Pureed, with attention to food allergies. Care Management will submit the request to Mom’s Meals and, if approved, two meals a day for 14 days will arrive at the member’s home packed in a cooler each week at no cost.
This benefit is as easy as it sounds and allows our members to focus on getting better after returning home from the hospital.
