Medicare Advantage –Diabetes and Congestive Heart Failure Care Management Programs

Our Medicare Advantage members with a diagnoses of Diabetes and Congestive Heart Failure (CHF), are eligible to enroll in a Diabetes/CHF Care Management Program. If eligible, they will be notified by Health New England, and a letter will be sent to their primary care physician.
The program allows members to be eligible for copay reimbursement for visits with their Primary care Physician, Endocrinologist, Cardiologist, Certified Diabetic Educator, and Nutritional Counselor. Members are required to pay their applicable copayment at the time of service, and reimbursements amounts will be automatically sent to members by Health New England at the completion of the program. Each program has a set limit that varies by plan.
