Note: Since the Insulin Reduction Act has been instituted, insulin is capped at $35 a month in all phases (deductible, initial, coverage gap, and catastrophic) and includes LIS beneficiaries. This is mandatory for all Medicare Advantage plans. Health New England is continuing to use Select Insulins in our printed formulary and formulary lookup tool as these insulins were submitted as part of our application for the Senior Savings Model. However, all insulins are capped at $35 even if they were not submitted as part of our application.
