These updates may apply to selected laboratory procedures and may include, but are not limited to:<\/p>\n<ul>\n<li>Diagnosis-to-procedure requirements (medical necessity validation)<\/li>\n<li>Procedure-to-procedure relationships and bundling requirements<\/li>\n<li>Frequency and utilization limitations<\/li>\n<li>Covered indication requirements<\/li>\n<li>Age, gender, and clinical criteria when applicable<\/li>\n<li>Documentation and coding requirements consistent with applicable coverage policies<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>Providers are encouraged to review applicable coverage criteria and ensure that submitted claims accurately reflect the patient&#8217;s clinical condition, supporting diagnoses, and services rendered.
