Healthmap\u2019s Kidney Health Management\u00a0(KHM) program works with your doctor to identify and support your health\u00a0needs, while helping you manage your chronic condition.<\/p>\n<p>Your doctor can recommend you for the program if you are at risk for CKD Stage 3 or higher level\u00a0of renal failure, including ESRD, which may require interventions such as hemodialysis or kidney transplant. This program is designed to help slow the progress of CKD and to prepare you if\u00a0interventions are needed.<\/p>\n<p>The KHM program is offered to you at no additional cost, and you\u2019re eligible\u00a0to receive Care Navigation services. Care Navigation consists of registered nurses and licensed\u00a0practical nurses working with you, in between doctor visits, to coordinate services, help answer\u00a0questions, provide education, and be your clinical support.
