We have partnered with Mom\u2019s Meals to provide meals to our Medicare Advantage members when they need it most: the days and weeks after coming home from an inpatient hospital stay. For 2021, this will be expanded to also include a stay at a skilled nursing facility.<\/p>\n
If you qualify, Mom\u2019s Meals will send up to 28 fully prepared, nutritious meals (up to 2 meals per day for up to 14 days \u2013 depending on your plan) to your home at no cost. Our Care Management team will contact you to assist with your request upon discharge. We will help you select a meal plan that fits your health needs and submit your request to Mom\u2019s Meals. This service must be requested within 30 days of discharge.<\/p>\n
