Caring Health Center has been innovative in their approach to meeting those needs. This is clear when you see their Wellness Center. The Wellness Center offers classes from chronic disease management to Zumba. Patients can come in and be part of their community, while feeling empowered to enhance their overall health and knowledge.<\/p>\n<p>High Street Health Center saw these same needs within their community. They saw the same vacancies, too. Staff from both centers met at the Wellness Center to discuss what this collaboration could look like. It was quickly apparent that our patients\u2019 needs\u00a0were the same as those of Caring Health Center\u2019s patients.<\/p>\n<p>We started by referring a small group of patients to the Chronic Disease Management course being offered. This course is designed to help people gain self-confidence in their ability to control their symptoms and learn how their conditions affect their lives. This course was\u00a0a great starting point, as we would be able to track the impact it would have on patients\u2019 health. Bridging the gap between health centers and giving our patients a place to connect\u00a0with others has had a positive impact.
