Improving the Patient Journey through Compassionate Connection and Teamwork<\/h3>\n

\u201cI have been a patient of Mason Square Neighborhood Health Center for about 18 years. During that time, I have been blessed with three primary care providers who have set the highest bar of care and concern. When I first started going in, it was my transition from childhood care into adult medicine. It had many unknowns for me. I had only one doctor my entire life. I remember people in the clinic helping me along the new process of scheduling my own appointments, keeping my insurance updated, and completing the health checklist that was required. This was all while I was in the midst of an undiagnosed\u00a0illness that was, and still is, very debilitating.<\/p>\n

\u201cThe Mason Square clinic has been serving the community so well, and for so long. They largely live amongst the people in the community, and they come in every day with the goal of uplifting the quality of life of their patients.\u201d \u2013 Zarinah Abdul-Rahim<\/p>\n

\u201cI\u2019ve been a patient at Caring Health Center for many years. I\u2019ve also been a part of the ACO PFAC since its launch in 2018. As both a patient and a member, I\u2019ve seen our health care leaders continue to share\u00a0clinical updates with our community.<\/p>\n

\u201cAs a representative of the refugee and immigrant community, I try to express opinions on how some clinical decisions impact our population. Bhutanese patients have many barriers to seeking health care. The pandemic has posed even more challenges for them to access the medical and mental health care
\nthey need.<\/p>\n

\u201cCaring Health Center has done a tremendous job at a crucial time. They understand the importance of community health workers and interpreters when delivering culturally-appropriate prevention and treatment messaging.
