How to Get Your $20 reward<\/strong>
\n\"\"<\/a>As a Medicare Advantage member, we will give you a $20 gift card for completing your 2019 annual preventive care visit. Once you\u2019ve had either your well-care visit or physical in 2019, log into our secure online Member Portal at my.healthnewengland.org and fill out a brief Preventive Care Visit Submission Form to receive your $20 gift card.*<\/p>\n

If you have questions, or aren\u2019t able to access our Member Portal to complete the form, please call our Member Services team who will help you at (413) 787-0010 or toll-free at (877) 443-3314\u00a0(TTY: 711) from 8:00 a.m. to 8:00 p.m., Monday through Friday (October 1\u2013March 31: 8:00 a.m. to 8:00 p.m., seven days a week).<\/p>\n
