As a Medicare\u00a0Advantage member, we will give you a $20 gift card for completing either\u00a0your annual well-care visit or annual physical exam in 2019.<\/p>\n<p><strong>How to Get Your $20 Reward<\/strong><br \/>\nOnce you\u2019ve had one of your free preventive care visits in 2019, log into\u00a0our secure online Member Portal at <a href=\"http:\/\/my.healthnewengland.org\" target=\"_blank\" rel=\"noopener\">my.healthnewengland.org<\/a> and fill out a brief Preventive Care Visit Submission Form to receive your $20 gift\u00a0card.* Form submission must be completed by December 31, 2019.<\/p>\n<p>If you have questions, or aren\u2019t able to access our Member Portal to\u00a0complete the form, please call our Member Services team who will help\u00a0you at (413) 787-0010 or toll-free at (877) 443-3314 (TTY: 711) from\u00a08:00 a.m. to 8:00 p.m., seven days a week.<\/p>\n<p><em>*Program limited to one reward per year and must be claimed by\u00a012\/31\/2019.
