Through education, the MTM Program helps you understand the appropriate use of your medications, and helps reduce the risk of potentially adverse events, such as falls, mental confusion and delirium.<\/p>\n<p>Our MTM Program is provided at no additional cost to you. If you qualify based on your pharmacy claims data, you will receive a Comprehensive Medication Review offer letter, which provides an overview of the MTM Program. The MTM program is voluntary and you can opt out at any time. We encourage you to take full advantage of this free service if you are selected.<\/p>\n<p>If you do get a letter, we ask that you please respond to either participate or opt out of the program. The MTM Program phone number is (800) 424-7885 and is available for you to call and schedule a Comprehensive Medication Review appointment with one of our clinical pharmacists or registered nurses, or to opt out if you\u2019re not interested.<br \/>\nLearn more about our MTM Program at <a href=\"http:\/\/healthnewengland.org\/medicare\/MTM\" target=\"_blank\" rel=\"noopener\">healthnewengland.org\/medicare\/MTM<\/a>.<\/p>\n<p><strong>Personal Medication List<\/strong><br \/>\nEven if you don\u2019t participate in the MTM Program, it is still a good idea to create a Personal Medication List to help you and your caregivers keep track of what medications you need to take.Having a Personal Medication List can help reduce medication<br \/>\nrisks and prevent medical errors.<\/p>\n<p><em>Some helpful tips to get you started:<\/em><\/p>\n<ul>\n<li>Create a Personal Medication List and list all your prescription medications, over-the counter drugs, herbals, vitamins and minerals. Keep this list up-to-date.<\/li>\n<li>Record any drug allergies or side-effects you have.<\/li>\n<li>Add new medications and record dates you started using them.
