That\u2019s why, as a Medicare beneficiary, you are entitled to an annual well-care visit with your primary care provider (PCP) once every 12 months. As a member of Health New England Medicare Advantage, your preventive care benefit covers both your annual well-care visit and your annual physical exam at a $0 copay.1<\/p>\n<h3>Don\u2019t Forget to Submit Your Well-Care Visit Form<\/h3>\n<p>If you have already had your 2018 annual well-care visit, please log into the Member Portal at my.healthnewengland.org and fill out a brief Annual Well-Care Visit Submission Form by\u00a0December 1st to receive a gift card. Remember to do the same thing next year once you\u2019ve completed your 2019 annual well-care visit. (Information submitted is subject to claims verification.)<\/p>\n<p>1The focus of the Annual Well-Care Visit is on prevention and screening, not detection and diagnosis.
