Schedule Your Annual Well-Care Visit in 2018<\/strong>
\nDid you know that the annual well-care visit is not the same as an annual physical exam? Your\u00a0annual well-care visit provides you with a valuable opportunity to talk with your doctor about your\u00a0health every 12 months, at no cost to you.<\/p>\n

During your annual well-care visit, you and your PCP can discuss the following topics and\u00a0develop a personalized prevention plan:<\/strong><\/p>\n

\u2022\u00a0 Your height, weight, blood pressure and\u00a0other routine measurements
\n\u2022\u00a0 Your medical and family history
\n\u2022\u00a0 Medications, supplements and vitamins\u00a0you are currently taking
\n\u2022\u00a0 Any allergies you may have
\n\u2022\u00a0 Health care you are receiving from\u00a0other providers
\n\u2022\u00a0 Your goals for maintaining good health\u00a0and independence
\n\u2022\u00a0 Your functional ability and activity level
\n\u2022\u00a0 Your ability to get around safely, in your\u00a0home and elsewhere (e.g., your risk\u00a0of falling)
