You and your PCP can discuss your medical history and develop a personalized prevention plan based on your current health and risk factors. Don\u2019t wait until you get sick to see your PCP; take advantage of your free annual well-care visit.<\/p>\n<p>As part of your Health New England Medicare Advantage plan, you have access to many preventive care benefits, which are designed to keep you healthy and most have $0 copays. The annual well-care visit is one of those benefits and is especially important for you and your PCP.<\/p>\n<p><strong>What is an annual well-care visit?<\/strong><\/p>\n<p>During your annual well-care visit, you and your PCP will go over the following:<\/p>\n<ul>\n<li>Your height, weight, blood pressure and other routine measurements<\/li>\n<li>Your medical and family history<\/li>\n<li>Medications, supplements and vitamins you are currently taking<\/li>\n<li>Any allergies you may have<\/li>\n<li>Health care you are receiving from other providers<\/li>\n<li>Your goals for maintaining good health and independence<\/li>\n<li>Your functional ability and activity level<\/li>\n<li>Your ability to get around safely, in your home and elsewhere (e.g., your risk of falling)<\/li>\n<li>Your emotional and mental health, and any concerns about depression, anxiety and memory loss<\/li>\n<li>Urinary incontinence<\/li>\n<li>Other concerns you may have about things like smoking cessation, weight loss, etc.<\/li>\n<li>A schedule for age- and gender-appropriate screenings and preventive services you need.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>The annual well-care visit is not the same as an annual physical exam, but it provides you with a valuable opportunity to talk with your doctor about your health every 12 months, at no cost to you.<\/p>\n<p><strong>Schedule your visit today<\/strong><\/p>\n<p>Call and schedule your annual well-care visit with your PCP today.
