If you have asthma or diabetes, or you struggle with mental health, Health New England\u2019s Care Management programs can help you get the information and care you need.<\/p>\n<p>We have care coordinators and case managers who will help you make decisions about your care. They can also help you stay connected with your doctors and care givers. We can also provide education and support for:<\/p>\n<ul>\n<li>Managing medications and costs<\/li>\n<li>Complex health needs<\/li>\n<li>High-risk maternity<\/li>\n<li>Depression, mental health, substance use disorder<\/li>\n<li>Asthma, chronic obstructive pulmonary disease, congestive heart failure, coronary artery disease, diabetes and hypertension.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>Care Management programs are available to all members at no cost.<\/p>\n<p><strong>How it works<\/strong><\/p>\n<p>You can be chosen for a Care Management program if we think you may have needs or by a doctor\u2019s referral. You can also self-refer. A care coordinator or case manager will then reach out to you to find out more about your needs.<\/p>\n<p>Once you enroll in one of the programs, you will work with one of our nurses or social workers through regular phone calls and check-ins. They will help you manage your health issues, identify resources and care givers, and get any referrals that you may need.
