\nIt\u2019s your opportunity to talk with your PCP about your overall health goals and concerns, and develop a prevention plan based on your current health and risk factors.<\/p>\n
\n\u2022 Medications, supplements and vitamins you are currently taking
\n\u2022 Health care you are receiving from other providers
\n\u2022 Your height, weight, body mass index, blood pressure, and other routine measurements
\n\u2022 Your emotional and mental health, and any concerns about depression, anxiety, memory loss or other cognition issues
\n\u2022 Your functional ability and activity level; and your ability to get around safely, in your home and elsewhere (e.g., your risk of falling)
\n\u2022 A schedule for screenings and preventive services you need
\n\u2022 Urinary incontinence and other concerns you may have like smoking cessation, weight loss, etc.
\n\u2022 Any risk factors and conditions you have and recommendations for treatment
\n\u2022 Health advice and referrals for health education or preventive counseling services or programs, as appropriate
\n\u2022 Advance health care planning and advance directives<\/p>\n
\nIt\u2019s an in-person head-to-toe exam that addresses your overall physical health, including the status of chronic or stable problems that may require follow-up; age-appropriate counseling, screenings and tests; measurement of blood pressure, weight, height and body mass index; a mini-cognition test and depression screening; and \u201cGet Up & Go\u201d testing.<\/p>\n
\nHealth New England offers Medicare Advantage plan members a $20 incentive for completing an annual preventive care visit.** To receive your $20 reward after you\u2019ve had your annual well-care visit or physical in 2024, log into our secure Member Portal at my.healthnewengland.org<\/a> to fill out the Preventive Care Visit Submission Form, or click here<\/a>.<\/p>\n
\n**Your PCP must bill this visit as an annual physical or well-care visit. Health and wellness assessments conducted in your home by our in-home health vendor do not qualify for the $20 reward. The 2024 program is limited to one reward per year and must be claimed by <\/em>12\/31\/2024.
