As part of our commitment to our members, Health New England partners with Healthmap Solutions\u00a0(Healthmap) to deliver comprehensive care for our Medicare Advantage members with chronic kidney disease (CKD) and end\u00a0stage renal disease (ESRD). Healthmap\u2019s Kidney Health Management (KHM) program works with your\u00a0doctor to identify and support your health needs, while helping you manage your chronic condition.<\/p>\n
Your doctor can recommend you for the program if you are at risk for CKD Stage 3, 4, 5 and ESRD,\u00a0which may require interventions such as hemodialysis or kidney transplant.<\/p>\n
This program is designed to help slow the progress of CKD and to prepare if interventions are\u00a0needed. The KHM program is offered to you at no additional cost, and you\u2019re eligible to receive\u00a0Care Navigation services. Care Navigation consists of registered nurses and licensed practical nurses\u00a0working with you, in between doctor visits, to coordinate services, help answer questions, provide\u00a0education, and be your clinical support. If your doctor recommends you for the program, you\u2019ll\u00a0receive information in the mail or via a phone call from a Healthmap Care Navigator.
