Reminder: Submit Your 2020 Preventive Care Visit Form by December 31st

We hope you have taken the opportunity to have an annual preventive care visit with your primary care provider (PCP) this year. As a Medicare Advantage member, we will give you a $20 gift card for completing either your annual well-care visit or annual physical exam in 2020.
How to Get Your $20 Reward
Once you’ve had one of your free preventive care visits in 2020, log into our secure online Member Portal at my.healthnewengland.org and fill out a brief Preventive Care Visit Submission Form to receive your $20 gift card.* Form submission must be completed by December 31, 2020.
If you have questions, or aren’t able to access our Member Portal to complete the form, please call our Member Services team who will help you at (413) 787-0010 or toll-free at (877) 443-3314 (TTY: 711) from 8:00 a.m. to 8:00 p.m., seven days a week.
*Program limited to one reward per year and must be claimed by 12/31/2020. Information submitted is subject to claims verification. Your PCP must bill this visit as an annual physical or well-care visit.
