New Program for Members with Kidney Disease

Because we work hard to provide the best possible care for our members, Health New England is partnering with Healthmap Solutions to deliver more comprehensive care for our Medicare Advantage members with chronic kidney disease (CKD) and end-stage renal disease (ESRD).
Healthmap’s Kidney Health Management (KHM) program works with your doctor to identify and support your health needs, while helping you manage your chronic condition.
Your doctor can recommend you for the program if you are at risk for CKD Stage 3 or higher level of renal failure, including ESRD, which may require interventions such as hemodialysis or kidney transplant. This program is designed to help slow the progress of CKD and to prepare you if interventions are needed.
The KHM program is offered to you at no additional cost, and you’re eligible to receive Care Navigation services. Care Navigation consists of registered nurses and licensed practical nurses working with you, in between doctor visits, to coordinate services, help answer questions, provide education, and be your clinical support. If your doctor recommends you for the program, you’ll receive information in the mail or via a phone call from a Healthmap Care Navigator.
