New for 2020: Post-Hospitalization Meals Program – Mom’s Meals

For 2020, Health New England has partnered with Mom’s Meals to provide meals to our Medicare Advantage members who are discharging from a hospital inpatient stay. Your well-being is important to us, and this new benefit is designed to support your nutritional needs when you most need it.
If you qualify, you can receive up to 28 fully-prepared, nutritious meals (2 meals per day for 14 days), delivered to your home at no cost to you. Our Care Management team will contact you to coordinate your request upon discharge. We will help you choose the nutritional option that is best for your health condition and we’ll submit your request to Mom’s Meals on your behalf. This home meal benefit must be requested within 30 days of discharge from an acute inpatient hospital.
