Mom’s Meals Helps Members After a Hospital Stay

We have partnered with Mom’s Meals to provide meals to our Medicare Advantage members when they need it most: the days and weeks after coming home from an inpatient hospital stay. For 2021, this will be expanded to also include a stay at a skilled nursing facility.
If you qualify, Mom’s Meals will send up to 28 fully prepared, nutritious meals (up to 2 meals per day for up to 14 days – depending on your plan) to your home at no cost. Our Care Management team will contact you to assist with your request upon discharge. We will help you select a meal plan that fits your health needs and submit your request to Mom’s Meals. This service must be requested within 30 days of discharge.
