I
f you have complex health needs and are on multiple medications, you may be eligible to participate in our Medication Therapy Management (MTM) Program, designed to help you and your doctor make sure that your medications are working. Through education, the MTM Program helps you understand the appropriate use of your medications, and helps reduce the risk of potentially adverse events, such as falls, mental confusion and delirium.
Our MTM Program is provided at no additional cost to you. If you qualify based on your pharmacy claims data, you will receive a Comprehensive Medication Review offer letter, which provides an overview of the MTM Program. The MTM program is voluntary and you can opt out at any time. We encourage you to take full advantage of this free service if you are selected.
If you do get a letter, we ask that you please respond to either participate or opt out of the program. The MTM Program phone number is (800) 424-7885 and is available for you to call and schedule a Comprehensive Medication Review appointment with one of our clinical pharmacists or registered nurses, or to opt out if you’re not interested.

Personal Medication List
Even if you don’t participate in the MTM Program, it is still a good idea to create a Personal Medication List to help you and your caregivers keep track of what medications you need to take.Having a Personal Medication List can help reduce medication
risks and prevent medical errors.
Some helpful tips to get you started:
- Create a Personal Medication List and list all your prescription medications, over-the counter drugs, herbals, vitamins and minerals. Keep this list up-to-date.
- Record any drug allergies or side-effects you have.
- Add new medications and record dates you started using them. Cross out medications you no longer use and record the date and why you stopped using them.
- Ask your doctors, pharmacists and other health care providers to update this list at every visit.
- If you go to the hospital or emergency room, take this list with you.
- Share this list with your family or caregivers.
- Keep the list in your wallet or purse.
