Matrix Home Health Assessments

Health New England has partnered with Matrix Medical Network. Matrix will be providing our Medicare Advantage Members with a valuable, no cost health assessment in the privacy of their home. The purpose of this visit is to keep our Medicare Advantage members as healthy as possible. Since 2001, Matrix has dedicated themselves to improving the health and lives of seniors. They work with Medicare Advantage plans throughout the United States.
The health assessment is meant to give members and their health care providers support in managing their medical concerns and their wellness. Matrix will work with HNE to help identify which members would most benefit from this assessment.
A highly trained Nurse Practitioner will conduct the assessment. He or she will meet with the member for about 60 minutes to assess current health and address any concerns. Each visit includes a limited physical exam, a complete review of medications, and a discussion of health history. The Nurse Practitioner schedules assessments on weekdays at a time most convenient for the member. A family member or caregiver also may be present.
The results of the assessment are confidential. They will be shared with HNE to ensure members receive the programs and services that would most benefit them. They also will be shared with the primary care physician to help him or her take better care of the member. Members may also request a copy.
This health assessment is not meant to take the place of the care received through regular doctor visits. While the program is completely voluntary, HNE encourages members to participate in this valuable program. Members who have participated in the home assessment have rated their experience positively.
