Schedule Your 2019 Annual Well-Care Visit and Physical Exam
Now is the time to get your Annual Well-Care Visit and Annual Physical Exam on the calendar for 2019. Preventive care and annual physical exams are important to staying healthy. That’s why, as a Medicare beneficiary, you are entitled to an annual well-care visit with your primary care provider (PCP) once every 12 months. As a member of Health New England Medicare Advantage, your preventive care benefit covers both your annual well-care visit and your annual physical exam at a $0 copay.1
Don’t Forget to Submit Your Well-Care Visit Form by Dec. 31
If you have already had your 2018 annual well-care visit, please log into the Member Portal at my.healthnewengland.org and fill out a brief Annual Well-Care Visit Submission Form by December 31st to receive a $10 gift card. Information submitted is subject to claims verification.
1The focus of the Annual Well-Care Visit is on prevention and screening, not detection and diagnosis. Please understand that if your physician addresses your additional medical concerns during this visit, that service will be subject to your PCP copay.
New for 2019
You can be rewarded with a $20 gift card for getting either your annual well-care visit or your annual physical exam in 2019. To request your gift card once you’ve completed your 2019 well-care visit or physical exam, log into the Member Portal at my.healthnewengland.org and fill out the Annual Well-Care Visit/Physical Submission Form (only one submission per member per year). The information you submit regarding your well-care visit or physical exam is subject to claims verification.
Reminder: Time to Get the Flu Vaccine
An annual seasonal flu vaccine is the best way to reduce the chance that you will get the flu and spread it to others. Receiving the flu shot may also reduce the risk of more serious flu outcomes if you do get sick. For more information about the flu, please visit cdc.gov/flu.
Health New England covers the flu vaccination at no cost to members when obtained from an in-plan provider. You can get a flu shot at your doctor’s office or participating retail pharmacy. Remember to consult with your primary care provider before receiving the flu vaccine.
