Do You Need Help Managing Your Medications?

If you have complex health needs, take multiple prescription drugs and have high prescription drug costs, you may be eligible to participate in Health New England’s Medication Therapy Management (MTM) Program, which is administered by our vendor partner Magellan Rx Management.^*

This program provides eligible Medicare Advantage members with a chance to review medications with a medical professional — either a pharmacist or registered nurse. The MTM program can potentially help you reduce your health care costs and your risk of medication-related problems.

This program is available at no charge and is included with your Medicare Part D benefits. To qualify, you must meet all three of the following criteria:

1. Fill at least eight different Medicare Part D
2. Have at least three out of the following five disease states: Alzheimer’s disease, bone disease (osteoporosis), chronic heart failure, diabetes, respiratory disease (chronic obstructive pulmonary disease (COPD)).
3. Estimate spending about $4,376 on medications over the course of the course of the year.
