The Healthy Directions Employer Wellness Program is comprehensive and engaging, focusing on building a culture of health at the worksite and improving employee health outcomes.<\/span><\/p>\n<p>There are four foundational components of the program:<\/p>\n<table border=\"1\" cellspacing=\"0\" cellpadding=\"0\">\n<tbody>\n<tr>\n<td valign=\"top\" width=\"343\">\n<p align=\"center\"><b>Assess<\/b><\/p>\n<ul>\n<li>Health Assessment\/WebMD Health Quotient<\/li>\n<li>Understand Current Health Status and Modifiable Health Risks<\/li>\n<li>Biometric Screenings\n<ul>\n<li>Total Cholesterol, HDL, LDL, and triglycerides<\/li>\n<li>Random blood glucose<\/li>\n<li>Blood Pressure<\/li>\n<li>Body Mass Index<\/li>\n<li>Body Composition<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/td>\n<td valign=\"top\" width=\"343\">\n<p align=\"center\"><b>Engage<\/b><\/p>\n<p>Programming concepts based on <i>Everything I Ever Needed to Know About Good Health, I Learned From My Mother<\/i>: <i>My Mother\u2019s Health Plan<\/i><\/p>\n<ul>\n<li>Quarterly Wellness Activities\/Events\n<ul>\n<li>Physical Activity<\/li>\n<li>Good Nutrition<\/li>\n<li>Prevention-based Care<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<tr>\n<td valign=\"top\" width=\"343\">\n<p align=\"center\"><b>Transform<\/b><\/p>\n<p>Based on Behavior Change Theories:<\/p>\n<ul>\n<li>Health Belief Model<\/li>\n<li>Transtheoretical Model (Stages of Change)<\/li>\n<li>Social Cognitive Theory<\/li>\n<li>Theory of Reasoned Action\/Planned Behavior<\/li>\n<\/ul>\n<\/td>\n<td valign=\"top\" width=\"343\">\n<p align=\"center\"><b>Maintain<\/b><\/p>\n<p>Incentive\/Reward Programming<\/p>\n<ul>\n<li>Participatory<\/li>\n<li>Outcomes-based (Health Contingent)<\/li>\n<\/ul>\n<p>Communications<\/p>\n<ul>\n<li>Personalized and Targeted<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>&nbsp;<\/p>\n<p>The program is implemented in phases that align with the four foundational components:<\/p>\n<p><b>Assess<br \/>\n<\/b><span style=\"line-height: 1.4em;\">The program begins with a Kick-off Event<\/span><b style=\"line-height: 1.4em;\"> <\/b><span style=\"line-height: 1.4em;\">at the worksite. The kick-off event compromises biometric screenings (blood pressure, blood glucose and cholesterol, triglycerides, and body composition), interactive health information, raffles and much more.\u00a0 It is a high energy and fun event that gets employees motivated to make an improvement in their health status.<\/span><\/p>\n<p><b>Engage<br \/>\n<\/b><span style=\"line-height: 1.4em;\">Two weeks post kick-off the Healthy Directions Health Coaches will implement the first activity challenge. This challenge is based upon the concept that everything you needed to know about good health you learned from your mother: go out and play (activity, exercise, stress relief, social health); eat your vegetables (importance of good nutrition); and wash your hands and brush your teeth (prevention). It is a progressive challenge, incorporating a new activity each week that builds upon the previous week throughout the month.<\/span><\/p>\n<p><b>Assess<br \/>\n<\/b><span style=\"line-height: 1.4em;\">WebMD Health Quotient is the next activity. This health assessment is available in an online environment as well as in a paper\/pencil format. Biometrics from the Healthy Directions Kick-off event will be automatically uploaded. Upon completion of the assessment a report is generated for each participant highlighting opportunities for health improvement. In addition, a baseline report utilizing aggregate data will be created and shared with the employer.<\/span><\/p>\n<p><b>Transform\/Engage<\/b><b>:<br \/>\n<\/b><span style=\"line-height: 1.4em;\">All Healthy Directions programming supports overall health improvement of your employee population. Programming is based upon clinical evidence and behavior change theory. In addition, programs are offered through a number of environments.<\/span><\/p>\n<p>Healthy Directions provides multi-modal programming opportunities:<\/p>\n<p>Online<\/p>\n<ul>\n<li>Digital Health Assistant<\/li>\n<li>Webinars<\/li>\n<li>Activities Challenges\u00a0 (individual)<\/li>\n<\/ul>\n<p>Onsite<\/p>\n<ul>\n<li>Kick-off Event<\/li>\n<li>Goal Setting Session<\/li>\n<li>Activity Challenges (individual and group)<\/li>\n<\/ul>\n<p>Community Based<\/p>\n<ul>\n<li>Walks, Runs, Cycling Events<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><b>Maintain<\/b><b><i><br \/>\n<\/i><\/b>\u201cMost of human nature runs on positive reinforcement and reward.\u201d Dee Eddington, Zero Trends.
