{"id":401,"date":"2010-03-18T11:13:57","date_gmt":"2010-03-18T15:13:57","guid":{"rendered":"http:\/\/www.hnetalk.com\/Provider\/?p=401"},"modified":"2010-03-18T11:13:57","modified_gmt":"2010-03-18T15:13:57","slug":"medicare-advantage-nurse-practitioner-home-visits","status":"publish","type":"post","link":"http:\/\/hnetalk.com\/provider\/medicare-advantage-nurse-practitioner-home-visits\/","title":{"rendered":"Medicare Advantage Nurse Practitioner Home Visits"},"content":{"rendered":"<p>Thank you for participating in our Medicare Advantage Network.<span> <\/span>I want to provide you with a description of our Clinician Home Visit Program for our Medicare Advantage members.<span> <\/span>With the Health New England (HNE) Medicare Advantage Plan, your patients have many benefits and services in addition to the traditional Medicare program. <!--more-->Our Clinician Home Visit Program, also known as the HNE Whole Health Assessment, is one of the many ways we support the needs of our members. As part of this home visit program, an HNE Medicare Advantage licensed nurse practitioner or physician assistant is available to visit your patient in their home or skilled nursing facility.<\/p>\n<p class=\"MsoNormal\">\n<p class=\"MsoNormal\">The results of this home visit, including laboratory results, will be provided to you and will assist in giving you a complete picture of your patient\u2019s health status. This home visit also will assist HNE in earlier identification of members requiring case management, disease management, and home care services. In addition, these visits will support HNE\u2019s Medicare Advantage correct coding initiative required for proper reimbursement of these members. While patient participation is optional, we encourage you and your HNE Medicare Advantage Plan member to take advantage of this program.<\/p>\n<p class=\"MsoNormal\">\n<p class=\"MsoNormal\">Your patients with HNE Medicare Advantage who have been identified for this program will receive a phone call from an HNE staff member within the next few weeks to schedule a time for the in-home visit. There will be no charge for this visit. This visit is not meant to take the place of any existing doctor appointments.<\/p>\n<p class=\"MsoNormal\">\n<p class=\"MsoNormal\">If you have any questions about this, please feel free to call our Provider Relations Department at 413-233-3313 or toll free at 800-842-4464 ext 3313.
