Members will have the opportunity to interact directly with a Registered Nurse who will work with the member to improve specific health-related goals. Available programs include, but are not limited to, Diabetes, Asthma, Coronary Heart Disease and High Risk Maternity. The Integrated Care case managers provide members with one direct contact at Health New England for medical concerns. This design allows them to build trusting relationships with members and treating providers. This document provides detailed information about the program and contains the annual workplan and schedule of events. It also explains how the program is evaluated.
