The following health plans now accept this form:<\/p>\n<p>&nbsp;<\/p>\n<ul>\n<li>Blue Cross Blue Shield of Massachusetts<\/li>\n<li>BostonMedicalCenter Healthnet Plan<\/li>\n<li>Celticare Health Plan of Massachusetts<\/li>\n<li>Fallon Community Health Plan<\/li>\n<li>Fallon Total Care<\/li>\n<li>Harvard Pilgrim Health Care<\/li>\n<li>Health New England<\/li>\n<li>Neighborhood Health Plan<\/li>\n<li>Network Health<\/li>\n<li>Tufts Health Plan<\/li>\n<li>Senior Whole Health<\/li>\n<li>Unicare<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><i>This form should <b><span style=\"text-decoration: underline;\">not<\/span> <\/b>be used to submit credentialing or contractual changes<\/i>. Continue to use individual health plan forms for those changes. \u00a0In some circumstances, individual health plans may need to follow up with providers for additional information regarding a demographic change.
