Programs and Policies Reminder
Posted on October 31, 2017
Below is a reminder about how to access key Health New England programs and policies:
Care Management Program
You can refer Health New England members with complex care needs to our Integrated Care Management (ICM) Program. Members will have the opportunity to interact directly with a Registered Nurse who will work with the member to improve specific health-related goals. Available programs include, but are not limited to, Diabetes, Asthma, Coronary Heart Disease and High Risk Maternity. The Integrated Care case managers provide members with one direct contact at Health New England for medical concerns. This design allows them to build trusting relationships with members and treating providers. For more information, call (800) 842-4464.
