Medical Policy Updates – Effective 9/1/2016

We have recently updated several Health New England medical policies that will become effective on 9/1/16:

• Clinical Trial for Medicare Advantage Members
• Clinical Review Criteria related to:
  • Behavioral Health Day Treatment
  • Behavioral Health Repetitive Transcranial Magnetic Stimulation
  • Continuous Glucose Monitoring
  • Genetic Testing
  • Implantable Miniature Ocular Telescope
  • Laser-Assisted Uvulopalatoplasty or Uvulopalatopharyngoplasty
  • Mobi-C Artificial Cervical Disc
  • Rhinoplasty
  • Total Hip Resurfacing

In addition, the following medical policies were revised, effective 6/14/16:

• Clinical Review Criteria Related to Biofeedback for Urinary Diagnoses
• Clinical Review Criteria Related to Fecal Microbiota Transplant
• Clinical Review Criteria Related to Orthognathic Surgery
