Drug Testing in Substance Use Disorder treatment and Pain Management
- Name changed from Drug Testing
- Minor changes to criteria
- No coding changes
Varicose Veins and Venous Insufficiency policy (effective 2/8/2024)
- Minor update to the ADL requirement
Trabecular bone score (effective 4/1/2024)
- 77089-77092 for trabecular bone score
- No longer Experimental & Investigational
- Not be separately payable
