Managed Care Enrollment, Primary Care Physician – New Online Form Available!
As part of a larger agency initiative to provide online resources and self-service options to members, the MassHealth Health Plan Enrollment Form is now available in two formats online. These self-service options are available for members who have been determined eligible for MassHealth Standard, CarePlus, CommonHealth, or Family Assistance and need to enroll in a health plan.
This is part of a multi-phase initiative that includes updates to the MassHealth website. This phase includes:
- The creation of three new tabs on the MassHealth homepage. These tabs are titled, “Apply for MassHealth,” “MassHealth Coverage Types,” and “Enroll in a Health Plan.” Each tab directs visitors to applicable web pages for more information.
- The MassHealth Managed Care Organization (MCO) Enrollment Form is now available online at www.Mass.Gov/MassHealth. Select the Enroll In a Health Plan tab on the left. From the Enroll in a Health Plan Page, members who have been program determined for MassHealth Standard, CarePlus, CommonHealth, or Family Assistance can view the list of plans available, enroll online in an MCO or PCC plan, or download the new paper enrollment form.
We encourage members who need to select an MCO/PCC plan to use these online resources wherever possible. The following questions and answers provide additional guidance:
Q: Can the online enrollment form be used to change MCO’s or to change from an MCO to the PCC plan? Are paper enrollment forms still required to switch health plans?
A: If a member wishes to enroll in a new plan, the member can complete and submit the online form.
Q: Is there a fax number that the MCO paper form can be sent to?
A: A fax number is not currently available. The member should complete the online form or complete and mail a paper form.
Q: Why does the confirmation screen not detail the selections made by the member?
A: There are rules regarding the display of PHI. This may be considered for a future release.
Q: What is the expected time frame for the MCO/PCC online selection to be updated in MMIS?
A: The forms are being processed within 1 business day of receipt.
