Signify Health In-Home Health Visits

Good health starts in the home. That’s why Health New England has teamed up with Signify Health to provide eligible Medicare Advantage members with a no-cost In-Home Health Evaluation. If you qualify, you will be contacted directly by Signify Health on behalf of Health New England.

This in-home health evaluation consists of meeting with a licensed clinician who will review your medications and medical history. They will also check your vital signs, look for safety risks, and answer your health questions. This visit does not replace your regular healthcare visit.
