Set Your Healthy Directions a NEW HNE Member Program!*

NOTE: This program is not available to all groups. If you’re not sure if Healthy Directions is available for you, please check with HNE.

This new HNE Healthy Directions program has been designed to help you become a master of your health. By completing a few simple health activities you will gain a better understanding of your health and learn ways to better manage it.

HNE wants to encourage and reward you for taking important steps to improve your health. The three step program includes having you complete the health activities outlined below, then will be entered into a monthly and year-end raffle.*

If a you believe that you are not physically able to complete any of the health goals within My Health Assistant, please contact the Health Management Program Department and a reasonable alternative will be provided.

Here is how the member program works:

STEP 1: Complete Your Health Assessment

Health Assessments are tools that can help you better understand your health risks. This may be the first step on your journey to a healthier direction.

How to complete your Health Assessment:

• First time users visit webmdhealth.com/hne and click
on Register.
• Returning users visit webmdhealth.com/hne and log in to begin.

Complete your Health Assessment for a chance to win a monthly and year-end raffle*!
• For every month after you have completed your Health Assessment, you will receive an automatic entry into a monthly raffle drawing for a chance to win a $100 Visa gift card.
• For an automatic entry into the year-end raffle you must complete your Health Assessment by December 31, 2014 for a chance to win a $500 Visa gift card.

STEP 2: Complete Your Biometric Screening

Biometric Screenings provide measurements of your health that can help you work towards maintaining or improving your health status.

How to complete your Biometrics Screening:

• Print the Biometric Screening Results Release Form and bring it to your provider to fill out.
• Mail the completed form to:

• When you have finished with your Biometric Screening, watch the Understanding our Biometrics webinar to learn more about what your biometric numbers mean.

STEP 3: Set Your Health Goals with My Health Assistant

You now have a good idea of your current health status. Set a Healthy Direction by using My Health Assistant.

How to set and complete Health Goals:

• Log into the Healthy Directions web-portal, at webmdhealth.com/hne and use the My Health Assistant to set and complete your Health Goals.

Complete your Health Goals for a chance to win a monthly and year-end raffle*!
• For every month after you have completed your first Health Goal, you will receive an automatic entry into a monthly raffle drawing for a chance to win a $100 Visa gift card.
• For an automatic entry into the year-end raffle you must complete your first Health Goal by December 31, 2014 for a chance to win a $500 Visa gift card.

* Restrictions apply
• You must be 18 years or older and have a valid Health New England Membership ID to be entered into any raffle.
• When registering, you must use your full 11 digit Member ID number including the last two numbers. (Example: 01, 02)
• Must have online access to participate in this program.
• Program applies to HNE’s Fully Funded group insurance membership only.
• Only health activities completed between January 1, 2014 and December 31, 2014 will
be accepted.
• You must be an HNE Plan Member on the date of the raffle drawing to be eligible for prizes.
• Drawing will occur at end of each calendar month, Winners will be notified within 2-3 weeks
from drawing.
