Schedule Your 2020 Preventive Care Visit

	As a Health New England Medicare Advantage member, your plan covers both an annual well-care visit and annual physical exam at no cost to you. Now is the time to schedule your free annual preventive care visit. We encourage you to call your primary care provider (PCP) today.
	Complete Your 2020 Preventive Care Visit and Get a $20 Reward* As a Medicare Advantage member, we will give you a $20 gift card for completing your 2020 annual preventive care visit. Once you’ve had either your well-care visit or physical in 2020, log into our secure online Member Portal at my.healthnewengland.org and fill out a brief Preventive Care Visit Submission Form to receive your $20 gift card.*
	If you have questions, or aren’t able to access our Member Portal to complete the form, please call our Member Services team at (413) 787-0010 or toll-free at (877) 443-3314 (TTY: 711) from 8:00 a.m. to 8:00 p.m., Monday through Friday (October 1–March 31: 8:00 a.m. to 8:00 p.m., seven days a week).

*Program limited to one reward per year and must be claimed by 12/31/2020. Information submitted is subject to claims verification. Your PCP must bill this visit as an annual physical or well-care visit.
