Schedule Your 2019 Preventive Care Visits Today

Now is the time to get your Annual Well-Care Visit and Annual Physical Exam on the calendar for 2019.

Preventive care and annual physical exams are important to staying healthy. That’s why, as a Medicare beneficiary, you are entitled to an annual well-care visit with your primary care provider (PCP) once every 12 months. As a member of Health New England Medicare Advantage, your preventive care benefit covers both your annual well-care visit and your annual physical exam at a $0 copay.1

Don’t Forget to Submit Your Well-Care Visit Form

If you have already had your 2018 annual well-care visit, please log into the Member Portal at my.healthnewengland.org and fill out a brief Annual Well-Care Visit Submission Form by December 1st to receive a gift card. Remember to do the same thing next year once you’ve completed your 2019 annual well-care visit. (Information submitted is subject to claims verification.)
