repetitive Transcranial Magnetic Stimulation (rTMS)

…is a noninvasive technique that is being investigated as a treatment for major depression. Brief pulses of magnetic energy are applied to the scalp via a large electromagnetic coil to generate low levels of current in underlying brain tissue. The goal is to stimulate areas of the brain involved in mood regulation to lessen duration or severity of depressive episodes.

The in-plan psychiatrist must complete discount Ultimate Obstacle Course the rTMS Prior Authorization Form and must enclose relevant clinical documentation to support this request:
Member must have a diagnosis of Major Depression; and
Member must be referred for rTMS by an in-plan psychiatrist; and
Member must have been in psychotherapy to address the depression; and
Member must have tried medication to address the depression with at least two adequate medication trials with antidepressants (of different types); andblow up water slides
Member must have been offered a trial of Electroconvulsive Therapy (ECT)
