Preventive Care: Your Annual Well-Care Visit*
Posted on March 20, 2017
Scheduling time to talk with your primary care provider (PCP) is a very important step in staying healthy. You and your PCP can discuss your medical history and develop a personalized prevention plan based on your current health and risk factors. Don’t wait until you get sick to see your PCP; take advantage of your free annual well-care visit.
As part of your Health New England Medicare Advantage plan, you have access to many preventive care benefits, which are designed to keep you healthy and most have $0 copays. The annual well-care visit is one of those benefits and is especially important for you and your PCP.
What is an annual well-care visit?
During your annual well-care visit, you and your PCP will go over the following:
- Your height, weight, blood pressure and other routine measurements
- Your medical and family history
- Medications, supplements and vitamins you are currently taking
- Any allergies you may have
- Health care you are receiving from other providers
- Your goals for maintaining good health and independence
- Your functional ability and activity level
- Your ability to get around safely, in your home and elsewhere (e.g., your risk of falling)
- Your emotional and mental health, and any concerns about depression, anxiety and memory loss
- Urinary incontinence
- Other concerns you may have about things like smoking cessation, weight loss, etc.
- A schedule for age- and gender-appropriate screenings and preventive services you need.
The annual well-care visit is not the same as an annual physical exam, but it provides you with a valuable opportunity to talk with your doctor about your health every 12 months, at no cost to you.
Schedule your visit today
Call and schedule your annual well-care visit with your PCP today. When making your appointment, be sure to ask for your “Annual Well-Care Visit” – which is different from an annual “physical exam.” At your visit, use the above checklist as a reminder for anything you wish to talk about.
