Falls Prevention Initiative for Medicare Advantage Members

As part of our commitment to helping our Medicare Advantage members effectively manage their health care, Health New England is pleased to offer a program focused on Falls Prevention. The goal of our Falls Prevention Program is to help you decrease your risk of falling and help you maintain your independence.
If you are at risk for falling, have fallen, or have an abnormality of gait diagnosis, you may be eligible to participate in this program.
As a participant, you will work directly with a qualified member of the rehabilitation team who will help to identify factors that could decrease your risk of falls, and provide you with a specialized strength and balance program.
Get Reimbursed
Once you have completed the program, the Falls Prevention rehabilitation team will submit a reimbursement request on your behalf to Health New England to reimburse you for the copayments paid during your visits, up to the maximum amount. This reimbursement process can take four to six weeks.

Please note: If you choose to participate in the Falls Prevention Program, you will require a referral from your primary care provider or specialist for the outpatient rehabilitation services.
