Community Health Center Creative Collaborations in Wellness

Baystate High Street Health Center and Caring Health Center have always been at the forefront of patient-centered care and valuing our community’s needs. Caring Health Center has been innovative in their approach to meeting those needs. This is clear when you see their Wellness Center. The Wellness Center offers classes from chronic disease management to Zumba. Patients can come in and be part of their community, while feeling empowered to enhance their overall health and knowledge.
High Street Health Center saw these same needs within their community. They saw the same vacancies, too. Staff from both centers met at the Wellness Center to discuss what this collaboration could look like. It was quickly apparent that our patients’ needs were the same as those of Caring Health Center’s patients.
We started by referring a small group of patients to the Chronic Disease Management course being offered. This course is designed to help people gain self-confidence in their ability to control their symptoms and learn how their conditions affect their lives. This course was a great starting point, as we would be able to track the impact it would have on patients’ health. Bridging the gap between health centers and giving our patients a place to connect with others has had a positive impact. We look forward to working together more in the future.
