Care Management Programs – Help for Long-Term Health Issues

If you have a long-term health issue, you know that a little help can go a long way. If you have asthma or diabetes, or you struggle with mental health, Health New England’s Care Management programs can help you get the information and care you need.
We have care coordinators and case managers who will help you make decisions about your care. They can also help you stay connected with your doctors and care givers. We can also provide education and support for:
- Managing medications and costs
- Complex health needs
- High-risk maternity
- Depression, mental health, substance use disorder
- Asthma, chronic obstructive pulmonary disease, congestive heart failure, coronary artery disease, diabetes and hypertension.
Care Management programs are available to all members at no cost.
How it works
You can be chosen for a Care Management program if we think you may have needs or by a doctor’s referral. You can also self-refer. A care coordinator or case manager will then reach out to you to find out more about your needs.
Once you enroll in one of the programs, you will work with one of our nurses or social workers through regular phone calls and check-ins. They will help you manage your health issues, identify resources and care givers, and get any referrals that you may need. They are here to help you with your overall health.

My health new England coverage will be ended on the 02.27 .2017 I am a diabetic person how can I get further coverage as i am unemployed and not working ,l am 64 year old with a wife and two (2) kids ages:7years old and 4years old they are covered but I will not be covered my date of birth is 08.05.53.
Thank you for your inquiry. You have a couple of options. You may qualify for MassHealth. You can either call MassHealth at (800) 841-2900, Monday – Friday, 8:30 a.m. – 5:00 p.m., or apply online through the Massachusetts Health Connector. Upon your 65th birthday, you may be eligible for Medicare. Three months prior to your 65th birthday, you can begin the Medicare enrollment process through Social Security. Social Security will determine the effective date of coverage.
