Annual Well-Care Visit Reminder*

Don’t forget about a very important preventive care benefit that you have as a Health New England Medicare Advantage member. Your Medicare Advantage plan covers an annual well-care visit at no cost to you. If you haven’t yet had your free annual well-care visit this year, we encourage you to call your primary care provider (PCP) to schedule an appointment.
During your annual well-care visit, you and your PCP can discuss the following:
- Your height, weight, blood pressure and other routine measurements
- Your medical and family history
- Medications, supplements and vitamins you are currently taking
- Any allergies you may have
- Health care you are receiving from other providers
- Your goals for maintaining good health and independence
- Your functional ability and activity level
- Your ability to get around safely, in your home and elsewhere (e.g., your risk of falling)
- Your emotional and mental health, and any concerns about depression, anxiety and memory loss
- Urinary incontinence
- Other concerns you may have about things like smoking cessation, weight loss, etc.
- A schedule for age- and gender-appropriate screenings and preventive services
Don’t wait until you get sick to see your PCP; take advantage of your free annual well-care visit. Call your PCP’s office to schedule your visit today, and bring this list of topics with you to discuss.
