Are you having an issue in your life that you could use some help with?\u00a0 HNE has care managers who are registered nurses and professional mental health care providers who are trained to help you with your needs.\u00a0 We also offer education programs for our members too!\u00a0 They help our members deal with chronic health conditions.\u00a0 You may learn some new information.\u00a0 These\u00a0education programs are\u00a0free and done in a series of up to 6 phone calls.\u00a0\u00a0During\u00a0the first phone call,\u00a0\u00a0you and the care manager complete an assessment.\u00a0 You then will receive a packet of information in the mail from HNE.\u00a0 We will give you time to review the information .\u00a0 Your care manager will then make arrangements to call you and answer all you questions that you may have about the educational information you received.\u00a0 You will then receive the next educational packet\u00a0and the process repeats itself.<\/p>\n
HNE offers educational programs for our members with asthma, diabetes and a healthy heart program.\u00a0 Parents my enroll in our asthma education program if their child is a member\u00a0 with asthma. We will teach you how to help you child deal with their asthma.\u00a0 Our care managers reach out to our members\u00a0on the phone\u00a0through our system.\u00a0 If you have not been contacted and wish to take advantage of this exciting opportunity simply call Health New England and request to speak to one of the care mangers today!<\/p>\n
