This NEW<\/strong> HNE Healthy Choices Member Program has been designed to help you become a master of your health. By completing a few simple health activities you will gain a better understanding of your health and learn ways to better manage it.<\/p>\n

HNE wants to encourage and reward you for making healthy choices to improve or maintain
\nyour health!<\/p>\n

Get Rewarded For Making Healthy Choices!<\/strong><\/span><\/p>\n

\"Healthy_Choices_Reward\"<\/a>Start earning points towards $25-$100 rewards when you complete the health activities outlined below.<\/em><\/strong><\/p>\n

\u2022 Complete your Health Assessment
\n\u2022 Complete Your Biometric Screenings
\n\u2022 Participate and successfully complete Health Activities for the 2015
\ncalendar year<\/p>\n

If a you believe that you are not physically able to complete any of the health goals within My Health Assistant, please contact the Health Management Program Department and a reasonable alternative will be provided.<\/p>\n

Here is how the Healthy Choices Member Program works:
\n<\/em><\/strong><\/span><\/p>\n

Step 1:<\/span> Complete your Health Assessment to earn points towards raffle prizes*!<\/strong><\/p>\n

– Members will be able to choose from different raffles from $25-$1,000 value.<\/p>\n

– Raffles will be drawn monthly, quarterly, and at the end of the year. Winners will be notified \u00a0\u00a0within 2-3 weeks from the drawing date.<\/p>\n

Step 2<\/span>: Complete your biometric screenings to earn points towards raffle prizes*!<\/strong><\/p>\n

– Members will be able to choose from different raffles from $25-$1,000 value.<\/p>\n

– Raffles will be drawn monthly, quarterly, and at the end of the year. Winners will be notified \u00a0\u00a0within 2-3 weeks from the drawing date.<\/p>\n

\n\u2022 You must be 18 years or older and have a valid<\/strong> Health New England Membership ID<\/strong>
\nto be\u00a0 entered into any raffle.
\n\u2022 When registering, you must use your full 11 digit Member ID number<\/strong> including the
\nlast two numbers.<\/strong> (Example: 01, 02)
\n\u2022 Must have online access to participate in this program.
\n\u2022 Program applies to HNE\u2019s Fully Funded and Self Funded group insurance
\nmembership only<\/strong>.
\n\u2022 Only health activities completed between January 1, 2015 and December 31, 2015
\nwill be accepted.
\n\u2022 You must be an HNE Plan Member on the date of the raffle drawing to be eligible
\nfor prizes.<\/strong>
\n\u2022 There will be a monthly, quarterly, and end of the year drawings. Winner will be notified within \u00a0\u00a0 2-3 weeks from the data of the drawing.<\/p>\n
