A member may be identified by:<\/p>\n<ul>\n<li>Complicated illness or multiple health risk factors<\/li>\n<li>Physician or provider referral<\/li>\n<li>Referral following an inpatient hospital stay<\/li>\n<li>Medical and pharmacy claims<\/li>\n<li>Member request<\/li>\n<\/ul>\n<p>An HNE Care Manager will screen referred members to assess medical, behavioral health, and social needs.
